The Better Practice Project is supporting the HACC sector in NSW to implement new models of care which support and enable clients to retain their independence and continue living in the community. Elton Consulting is working with partner Age Communications to conduct an independent evaluation of components of The Better Practice Project. As part of this, we are evaluating four demonstration projects which are trialling a new short-term intensive enabling approach with HACC clients.
The evaluation team is working with the demonstration projects to maximise opportunities for stakeholder learning, growth and change during The Better Practice Project and beyond. We are also working closely with the Department and sector to ensure evaluation findings and lessons help to form and shape enablement practice in NSW over the next 12 months.
